How a referral works
Call or fax us with the patient's details. We take it from there — including chasing any missing orders or documentation, so it does not come back to your desk.
- Call or fax the referral to our office
- We verify insurance coverage and confirm eligibility
- A registered nurse completes the in-home assessment
- We report back to the referring physician and begin the plan of care
What we need from you
- Patient name, date of birth and contact details
- Physician's order and current plan of care
- Relevant diagnoses, medications and recent discharge summary
- Insurance information
